Your Generation Care Pty Ltd is an NDIS registered disability support provider in Adelaide's north-east, with an office in Walkerville and its head office in Windsor Gardens. The team supports children, young people and adults with intellectual, physical and sensory disabilities or psychiatric conditions, including people with complex and multiple disabilities and people who are ageing with disability. Support is guided by each person's aspirations for the life they want to lead, with a commitment to human rights and self-determination and values of compassion, empowerment, respect, excellence, diversity and inclusion, integrity and collaboration.
Registered supports cover assistance with daily personal activities including high intensity supports, community nursing care, daily tasks in shared living, household tasks, development of life skills, assistance with travel and transport, participation in community, social and civic activities, group and centre based activities, innovative community participation, assistance with life stage transitions, accommodation and tenancy assistance, Supported Independent Living and Specialist Disability Accommodation.
A rapid hospital discharge programme helps participants return home with the right supports in place. Referrals are accepted from participants, families, carers, support coordinators and health professionals through the website's referral form, followed by an initial assessment, collaborative planning and regular reviews. The team is available Monday to Friday, 9 am to 6 pm, and respects and honours the history of Aboriginal and Torres Strait Islander peoples.
