We offer support to people living with a disability on a smaller scale basis where an emphasis on providing exceptional person centered care and communication is second to none! We pride ourselves on delivering quality, person centered and goal orientated services that empower you to live your best life. Our Specialist Support Coordinators have a higher level of qualification, skills and experience within a particular area (Housing, Mental Health, Complex Supports, Dual Disability, Child Protection, Justice and Early Childhood Intervention) to provide intensive, usually time limited assistance to help you achieve your goals. An area of expertise is ALL THINGS HOUSING - SDA, MTA, STA, SIL, Respite and innovative living options. We know how complex and difficult it is to navigate finding a new home and transitioning into your new home. We are here to help every step of the way. Our Support Coordinators are friendly and caring professionals who will support you to understand and implement your NDIS plan to achieve your goals. Some tasks we can support you with include find the right services out there in the community who meet your needs, completing referral paperwork, building your capacity (so that perhaps someday you may not need us), money and budgeting, preparing for plan reviews and progress reporting to NDIA. Our Service Locations: We provide services across Australia. We also offer virtual services for participants who are busy or live in remote locations.
