Specialist Support Coordination (Level 2 & 3) | Complex, High-Risk & Hospital Discharge | National Telehealth Clinical Oversight 🩺 The Virtual Care Collective is a clinician-led national telehealth NDIS provider delivering Support Coordination Level 2 and Specialist Support Coordination Level 3 for participants with complex needs, high-risk environments and multi-disciplinary care requirements. Led by a senior Registered Nurse with nearly 20 years’ experience across emergency care, mental health, paediatrics, remote healthcare and clinical governance, we provide structured, risk-informed Capacity Building support within the NDIS framework.

Specialist Support Coordination & Clinical Oversight We engaged The Virtual Care Collective for Specialist Support Coordination (Level 3) due to complex needs involving mental health, multiple providers, medication considerations, and safeguarding concerns. Ryan brought a clinician-led approach that immediately improved clarity and structure within our NDIS plan. This was not standard Support Coordination. He provided clinical navigation, coordinated communication between our GP, psychiatrist, allied health providers and support team, and ensured medication safety concerns were escalated appropriately without working outside scope. Risk factors that had previously been overlooked were identified and addressed. Clear documentation supported our NDIS funding, and capacity building goals were aligned to real-world outcomes. Hospital transition planning and provider integration were handled with professionalism and strong clinical governance principles. What stands out most is the level of oversight and systems thinking.
