Quality Care Network

Psychosocial Recovery Coaching, Support Coordination, Specialist Support Coordination

NDIS Registered

From the provider: Quality Care Network is an independent NDIS registered provider specialising in Support Coordination, Specialist Support Coordination, Psychosocial Recovery Coaching, and strategic social work supports for participants with complex needs. Our team is built around a simple philosophy: people deserve support that understands real life. Many of our practitioners bring lived experience of disability, mental health recovery, or navigating the service system, alongside formal qualifications in social work, community services, and mental health peer work. This allows us to combine professional expertise with genuine empathy and understanding. We work with participants across Victoria and beyond who may be living with psychosocial disability, trauma, neurodivergence, or complex life circumstances. Our focus is on helping participants build capacity, navigate the NDIS system, and create sustainable support networks that improve long-term wellbeing. Our services include: • Support Coordination – helping participants understand and implement their NDIS plan • Specialist Support Coordination – supporting people with complex barriers and high-risk situations • Psychosocial Recovery Coaching – recovery-focused support for people living with mental health challenges • Allied health collaboration, including dietitian services where appropriate We take a trauma-informed and recovery-oriented approach, meaning participants remain at the centre of every decision. Our role is to walk alongside people as they build independence, strengthen their support systems, and move toward their personal goals. At Quality Care Network, we believe the best outcomes happen when support is respectful, flexible, and deeply human.

Support Coordination Network

Specialist Support Coordination

NDIS Registered

From the provider: The Support Coordination Network was founded in 2012 by a team of people who have accumulated a wealth of knowledge with over a decade of working within the community services sectors and they identified the need to create an innovative, outside of the box and outcomes driven service! We offer support to people living with a disability on a smaller scale basis where an emphasis on providing exceptional person centered care and communication is second to none! We pride ourselves on delivering quality, person centered and goal orientated services that empower you to live your best life. Our Specialist Support Coordinators have a higher level of qualification, skills and experience within a particular area (Housing, Mental Health, Complex Supports, Dual Disability, Child Protection, Justice and Early Childhood Intervention) to provide intensive, usually time limited assistance to help you achieve your goals. An area of expertise is ALL THINGS HOUSING - SDA, MTA, STA, SIL, Respite and innovative living options. We know how complex and difficult it is to navigate finding a new home and transitioning into your new home. We are here to help every step of the way. Our Support Coordinators are friendly and caring professionals who will support you to understand and implement your NDIS plan to achieve your goals. Some tasks we can support you with include find the right services out there in the community who meet your needs, completing referral paperwork, building your capacity (so that perhaps someday you may not need us), money and budgeting, preparing for plan reviews and progress reporting to NDIA. Our Service Locations: We provide services across Australia. We also offer virtual services for participants who are busy or live in remote locations.

Thrive Functional Health

Support Coordination, Specialist Support Coordination, Physiotherapy

From the provider: Thrive Functional Health delivers NDIS physiotherapy and support coordination under one roof — combining hands-on clinical expertise with the practical know-how to help participants get the most from their plan. As a registered NDIS provider, we work with people across the lifespan, including those living with neurological conditions (stroke, Parkinson's, MS, acquired brain injury), chronic and acute pain, post-surgical recovery, falls and mobility concerns, and complex or lifelong disability. Our physiotherapy is evidence-based, goal-oriented and active. Rather than passive, short-term treatment, we focus on functional movement — the everyday abilities that matter most to you: walking further, transferring safely, climbing stairs, returning to work or sport, and staying independent at home. Every participant begins with a thorough functional assessment, followed by a tailored program that blends manual therapy, targeted exercise, strength and conditioning, and education so you understand your body and can manage your condition with confidence. We can see you in the clinic, in your home or out in the community — whatever suits your goals and mobility. We set clear, measurable goals aligned to your NDIS plan, track your progress, and provide the detailed assessment and progress reports your planner or plan reviewer needs at review time. We collaborate closely with GPs, specialists, allied health teams and support workers to keep your care coordinated and consistent. Our support coordination helps you turn a funded plan into real-world outcomes. Navigating the NDIS can be overwhelming — understanding your budget, choosing providers, arranging services and preparing for your plan review all take time and know-how. Our coordinators work alongside you to build your understanding of the scheme, connect you with quality mainstream and community supports, negotiate and set up service agreements, resolve issues as they arise, and strengthen your capacity to manage your own supports over time. Whether you need Support Connection to get started or ongoing Coordination of Supports for a more complex situation, we tailor our involvement to your goals and your funding — and because our team includes qualified allied health professionals, we bring genuine clinical insight to the way your supports fit together. As your review approaches, we help you gather evidence and articulate your goals so you walk in prepared. Bringing physiotherapy and support coordination together means your clinical care and your broader supports actually talk to each other. Your physiotherapist's goals inform how your plan is coordinated, and your coordinator understands the therapy behind your progress — a joined-up approach that saves you repeating your story and helps your plan work harder for you. Where we provide both services, we're transparent about it and always protect your right to choose your own providers, in line with NDIS conflict-of-interest requirements. Thrive supports participants who are plan-managed, self-managed and NDIA-managed, and we welcome referrals from participants, families, support coordinators, plan managers and other providers. Our promise is simple: skilled, respectful, participant-first care that helps you move better, live more independently and get the most from your NDIS plan. TIDE Community Care

Specialist Support Coordination

From the provider: TIDE Community Care is built around Specialist Support Coordination — the centre point where complexity is understood, risks are managed, and systems are brought into order. Our coordination model leads the entire service response, ensuring that NDIS plans translate into real‑world outcomes, providers stay aligned, and participants remain safe, stable, and supported in their homes. From this foundation, TIDE integrates the practical supports that keep people well. Environmental control is our first layer of risk management, delivered through domestic assistance, lawn care, and trauma‑informed or biohazard cleaning when required. These services protect safety, access, and tenancy, forming the groundwork for stability. Personal supports provide daily structure — assistance with activities of daily living, community access, routines, and high‑intensity supports where clinically appropriate. Our clinical services strengthen the system further, offering high‑intensity nursing, assessments, continence management, mental‑health‑informed care, and formal reporting aligned with NDIS and aged care standards. Education underpins everything we do, building capability across clients, families, and support teams so decisions are informed and supports remain sustainable. TIDE is not a list of services — it is a coordinated, clinically informed response that holds under pressure, reduces risk, and restores order for people whose needs exceed standard service models.
