Northern Rivers Support Coordination Services is a sole-trader NDIS provider based in Lismore that supports adolescents and adults to use their NDIS funding well. It offers support coordination, specialist support coordination for people with complex or high-risk needs, and psychosocial recovery coaching for people with a primary psychosocial disability. Coordinators help participants manage plan budgets, understand what is funded, and connect with clinical, housing, medical and vocational supports.
Specialist work begins with a risk matrix, completed with the participant, that sets out risks, who they may affect and the strategies needed.
