XtremeCare Australia is a privately owned NDIS registered provider that began operating in 2018 with a focus on First Nations communities. It delivers Support Coordination and Plan Management, and its allied health team provides physiotherapy and occupational therapy. Sessions take place in clinic, through outreach and mobile visits, and by telehealth, including for participants in remote Queensland locations such as Weipa and the Western Cape. The provider employs local community members, works alongside health, education and housing services, and helps participants build the skills to coordinate their own supports.
Support Coordination
Helps participants understand and use their NDIS plan, connect with providers and community services, and build the skills to coordinate their own supports.
