Specialised Care Coordination

Specialised Care Coordination is an NDIS support coordination provider working with people who have disabilities and mental health conditions. The team helps participants understand their NDIS plan and budgets, choose and connect with service providers, coordinate supports and service schedules, and set up service agreements and bookings. Coordinators coach participants to build confidence and skills so they can implement their plan themselves, and support people moving on from hospital, custody or community mental health services. The managing director brings more than ten years of experience in disability and mental health, and coordinators speak Arabic and Swahili alongside English.
