Support coordination helps you to make the best use of your supports in plan. Support Coordination is a capacity building support which helps you to: 🙂Understand and use your NDIS plan to pursue your goals 🙂Connect you with NDIS providers, community, mainstream and other government services 🙂Build your confidence and skills to use and coordinate your supports. As Support Coordinators we are attentive and work collaboratively with all Participants to achieve the following; • To engage service professionals for assessments and training to support the service user.
• To create Service Agreements between the Participant and chosen service. • To explain the NDIS pricing guide and advise Participant and family. Always looking at the overall finances for the Participant and allocating funds accordingly. • To assist the Participant and their family, how to problem solve issues that might present. • To educate Participants and families of their responsibilities under a service agreement.
• To change or amend a Service Agreement. • Provide an 8 week report and a 9 month report to inform NDIS what has worked and what hasn’t. This includes the Participant for their next NDIS plan review.
