Pal Support Hub is a service that shows empathy and integrity towards people who have a disability. My service provides support as a Social Educator. I am registered with NDIS. I do community access for most of my participants. I provide a service for participants who are on plan managed or self managed. My aim is to focus on a person-centred approach. It is important to ensure that the focus is on what matters to the people receiving support to improve quality of life.
My first consultation is identifying what the participants goals are? and what outcome do they want to achieve? My role is to take out participants to where they want to go. I assist them with social activities, Schedule appointments, assist with shopping, art and crafts and day excursions etc. I pick participants up from their home using my own vehicle. I also assist participants who have limitations with reading and assist them with Telehealth and Zoom.
I assist with improving their life skills and I do small domestic duties. The aim is to give people who have a disability the confidence and skills towards independence. I am organised and i like to put a program for my participants so they can review their goals and progress ( if applicable). I also make sure each of my participants carry a diary for their own personal reporting and for safety reasons if they use other services with other people.
From my experiences and my qualifications, while working in the service sector. I lead by example by providing services fairly. I am able to not be bias on participants circumstances and focusing on client’s individual needs.
