National Disability and Mental Health Services (NDMHS) is a Registered NDIS Provider. Our team of specialised staff have extensive experience in support coordination and specialised support services for people with psychosocial disabilities, complex physical illness, intellectual and mental health diagnoses and aged care.
We believe in person-centred services and recovery-based practices . Our referral processes and systems have been streamlined for speed and efficiency. We understand the importance of service delivery for our client’s daily lives and overall wellbeing therefore we offer no waiting periods for urgent needs. We will ensure we have a support coordinator on hand for immediate service delivery in these circumstances. Our diverse team of health, social and environmental workers specialise in providing tailored support to assist our clients in their own home and with community-based activities.
Your Support Coordinator will closely work with you to access the services specific to your requirements as well as your future goals and personal interests. We also offer an eligibility assessment service for people wanting to access the NDIS. NDMHS Services include: SUPPORT COORDINATION | The process of obtaining and utilising your NDIS plan can sometimes be tedious and confusing with so many choices now available. To ensure you are getting the most out of your plan, your Support Coordinator will provide their professional assistance and guidance for linking you with the services that suit you!
We will work with you so you actually understand your plan and its implementation. We will also provide support for scheduled and unscheduled reviews. Your Support Coordinator will advocate to the NDIS either on your behalf or with you which ever you prefer to ensure you get the support and services that you need. SUPPORT WORKERS SERVICES: SOCIAL & COMMUNITY SUPPORT | Our team of culturally diverse Support Workers have been professionally trained to provide person-centred service delivery to our clients.
Our in-house methods involve a strengths-based approach, focusing on a client’s abilities and working within their constraints to build resilience and increase capacity socially as well as in their home environment. We focus on empowerment, empathy, acceptance and high-quality, person-centred service provision. IN-HOUSE SUPPORT | To ensure our clients maintain a healthy and conducive home environment, our team of support workers can also provide domestic assistance with routine cleaning, grocery shopping, laundry and ironing services as well as food preparation.
PERSONAL CARE | For some aged care clients as well as people living with certain physical or mental disabilities maintaining proper hygiene and self-care routines can be a challenge. Our support workers can assist with showering, toilet use, continence care, grooming, dressing and transferring in and out of bed. TRANSPORTATION & TRAVEL SUPPORT | We can provide transportation for clients with medical appointments and personal life events. Our support workers are industry-trained in travel support and our vehicles have been customised with the necessary equipment to provide clients with maximum comfortability and safety.
All vehicles have also been had fitted with emergency equipment. ENVIRONMENTAL MAINTENANCE | NDMHS has a dedicated team to provide services to clients requiring assistance with their lawns and gardens. These staff are trained in industry standards ensuring at all times the safety of our clients and staff are paramount. PSYCHOSOCIAL RECOVERY COACHES | What is a Psychosocial Disability? ’ Diagnoses include: Bipolar Disorder Schizophrenia Obssessive Complusive Disorder (OCD) Severe and persistent anxiety and depression And more!
As of 1st July 2020 a new support system called ‘Psychosocial Recovery Coach’ became available to all NDIS participants living with a psychosocial disability. This service is for those clients requiring support managing complex challenges in their daily lives. The aim is to increase clients’ social and economical independence and participation through the provision of capacity building supports. NDMHS Recovery Coaches will focus on coaching clients to have a greater participation in managing their daily lives.
We will collaborate with other services to support and underpin these outcomes. Recovery Coaches work collaboratively with clients, their family and carers and other services to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports. Recovery Coaches work on clients dreams, hopes and aspiration using strengths-based approaches to allow clients to live full and meaningful life despite their disability and the daily challenges they may face.
PHYSIOTHERAPY | Our team of highly trained Physiotherapists use a variety of specialised treatments for the management of pain and physical impairment. Our methods have been medically tested and approved by the NDIS for their effectiveness.
