We are a family-owned business that has extensive experience in the disability, mental health and community sector. We have been supporting people within the community for over 20 years. We are funded through NDIS plans to provide support to individuals and families to coordinate services and implement their plans. Our job is: • To help source and engage service providers for direct supports. • To help source and engage professionals for assessments and training to support the participant.
• To help develop service agreements between the participant and their chosen service. • To help develop a participant’s NDIS budget based on the services and supports they need. • To educate participants and families around the NDIS pricing arrangements and line items used in NDIS invoices. • To assist the participant and their family with problem-solving any issues that might arise. • To educate service users and families about their responsibilities under a service agreement.
• To assist with changing or amending service agreements. • To assist the participant to link in with other informal and mainstream community activities and services. • To help the participant empower themselves to progress towards their goals by offering choice and control. • To help liaise with service providers, the NDIA and other government agencies as required. • Prepare the participant for their next NDIS plan review or reassessment.
