Healthproof supports participants across a broad range of funding types and care pathways, including the NDIS, aged care (including home-based and facility support), private clients, and other referral streams. We work closely with participants’ support networks, including families, support coordinators, care managers, case managers, and medical professionals, to ensure therapy aligns with individual goals and delivers meaningful, measurable outcomes.
Our multidisciplinary team provides mobile services across Sydney and surrounding regions, with capacity to support participants through Telehealth where clinically appropriate.
