I have over 30 years working in the disability sector in roles including direct support, case management, mental health support, employment support, service management, NDIS planning as a Senior LAC & Service Manager. I am very familiar with working with people with high physical support needs after managing statewide in home supports services for 11 years. I have always enjoyed supporting people to work towards the outcomes they want for their lives.
That is why I decided to become a Support Coordinator. NDIS support coordination is about supporting people to understand their plan and find the most appropriate resources to realise their goals. I have supported NDIS participants with: - Referrals to allied health. - Accessing Short Term Accomodation. - Applying for SDA & SIL. - Applying for and preparation for plan reassessment. - Working with mainstream services such a Centrelink, Medicare, Court Hearings etc.
- Care Team Meetings inlcuding Families, Therapists, Schools and others as required. - Source and referral to social and recretional activities. - Source and manage support workers. - Monitoring plan budgets. - Complex issues such as dual diagnoses, justice involvement, homelessness, young people in nursing homes. - Other tasks as they arise.
