Doltak Care is a nurse-led NDIS provider based in Wellard, Western Australia, founded and led by a Registered Nurse. It supports people with disability to live more independently at home and in their communities, offering personal and daily living assistance, high intensity personal care, community nursing, supported independent living, household tasks, transport, group and centre activities, community participation, plan management and meal preparation.
Support is delivered in participants' own homes and in shared or individual accommodation, with staff matched to each person's preferences and routines. The team works with participants, families and support coordinators on goals, budgets and life stage transitions such as leaving school or moving home.
