Connect Case Management (ccm.) is a NDIS-registered service provider based in Perth, Western Australia. We make support coordination, therapy and training accessible to everyone. Our team aren’t just support coordinators and case managers, they’re leading health and social care professionals. Our services are founded on person-centered planning. By working closely with clients, their families, and partners, we foster enduring and meaningful support.
We offer Specialist Support Coordination, Occupational Therapy reports (Functional Capacity Assessments and NDIS Home and Living Reports), and Case Management services. Our Support Coordinators and Case Managers are health and social care professionals with expertise in neurological conditions, spinal cord injury, mental health, and complex case management. We also offer training workshops which aim to develop the knowledge of Acquired Brain Injury (ABIs) for those working in the industry.
