Alive4Life is an NDIS registered disability service provider based in Campbelltown, New South Wales. It was founded by Kelly Middlebrook and offers plan management, support coordination and specialist support coordination to participants across Australia, alongside direct supports delivered locally. Direct supports include supported independent living and specialist disability accommodation guidance, respite and short-term accommodation, adult day programs, after-school and school holiday programs, personal care, household cleaning, lawn and garden care, community transport, counselling, art therapy and functional capacity assessments.
Day programs and school holiday activities run from the provider's Campbelltown wellness centre in small groups, with community outings, and the team includes people with backgrounds in health, business and community services.
