I have been working in Primary Care and the NDIS space for the past ten years. Here are some of my roles: Supporting and maintaining clients in the community as far as possible, · Helping client’s access community resources,· Working with clients to improve and maintain their mental health, · Educate and monitor for early warning signs that may indicate a mental illness episode   to be able to minimize it’s impact and help prevent hospitalization · Working in the home with client’s who are unable to leave their home or require home interventions · Helping clients with strategies to manage stress (which is a major factor in recurring episodes of illness) and their mental illness · Teaching communication, assertion, and people skills to help maximise client’s integration into the community · Attending GP and Psychiatry appointments with clients and supporting if they’re unable to express their needs.
