Konnectus Support Coordination is an NDIS support coordination provider and part of the My Way family. It delivers Level 1 and Level 2 support coordination, specialist support coordination, psychosocial recovery coaching and home and living supports in Western Australia, Victoria and the Northern Territory. Coordinators help participants understand their plans, connect with providers and mainstream services, arrange service agreements and prepare for plan reviews. Specialist coordinators work with people in complex circumstances such as hospital discharge, justice system involvement and housing applications. The team includes people from a range of cultural backgrounds.
