We will meet with you to assess if your goals are being achieved, if you require any changes to services, or if your circumstances have changed. During our Support Coordination meetings together, we will exercise your choice and control to ensure the following: Identify services that match your individual needs; Organise referrals, select appropriate service providers, establish service agreements, as well as organising the timing of your supports; Monitoring of the delivery of supports; Support you to liaise with other government agencies as required; Assist you to link with community activities and employment; Coordinate with health services; Monitoring of your NDIS plan outcomes, expenditure and reporting back to the National Disability Insurance Agency (NDIA).
