About Us Project8 Support Coordination sets out to give every person a chance to be heard and treated equally within our society. We foster core values, attitudes, and culture of equality, inclusion, empowerment and most importantly support. Many people with a disability are now funded under the National Disability Insurance Scheme (NDIS), but still without adequate support systems in place. Project8sc supports the National Disability Insurance Agencies values, capabilities and behaviours.
Project8sc sets out to align with these through our own core values, attitudes, and culture to make a difference, and to do our part in the community to provide a solid foundation of support and linkage. We want every person to have an opportunity to live great lives, to be included in our community, to be treated fair and equal, to be empowered to make their own choices and feel supported by everyone that touches their lives. What will Project 8 Support Coordination do for me? We will meet with you to assess if your goals are being achieved, if you require any changes to services, or if your circumstances have changed. This is to guarantee you are continually receiving the best support. During our Support Coordination meetings together, we will exercise your choice and control to ensure the following: Identify services that match your individual needs; Organise referrals, select appropriate service providers, establish service agreements, as well as organising the timing of your supports; Monitoring of the delivery of supports; Support you to liaise with other government agencies as required; Assist you to link with community activities and employment; Coordinate with health services; Monitoring of your NDIS plan outcomes, expenditure and reporting back to the National Disability Insurance Agency (NDIA).
We will assist you to redevelop your goals prior to your plan review. Our focus is on you. Your aspirations and strengths are of the highest importance. We will tailor our supports to you ensuring the highest quality of Support Coordination. We will walk beside you during your NDIS journey making sure your choice and control has been exercised to live your life to its fullest potential. That is our commitment to you. Policy Project8 Support Coordination follows the same NDIS Quality and Safeguards Commission Practice Standards and NDIS Code of Conduct as a registered provider would.
The NDIS Code of Conduct applies to any provider or individual who is registered or unregistered intending on delivering services to any NDIS Participant. This guarantees the safe and efficient delivery of services to you.
