From the provider: We are registered NDIS provider, having more than 15 years of nursing experience in community health nursing, disability sector, AOD, mental health, specialised support coordination, training and assessment- nursing and mental health - looking for NDIS clients in Melbourne( western and eastern suburbs), Geelong & Lara to start providing following services- Community Nursing services-Through the community nursing care program, participants are trained on how to take care of their health in scenarios such as medication, wound care, hygiene, and help with showering or dressing. There is usually a registered nurse or medical practitioner appointed who will monitor the patient’s progress and will amend the treatment and medication accordingly. Here are some of the services offered under the community nursing care program: Community Nursing Care is provided to participants who has complex individual support needs that are best met by someone who has medical training and experience to meet the participants’ complex health needs. Our NDIS community nursing care is needed when specific individual support needs include: Very limited communication Behaviour support needs, and/or Medical needs / procedures such as ventilation or home enteral nutrition Kindly Care Australia is Registered NDIS Provider for Community Nursing Care. We are registered to deliver: Complex Bowel Care Enteral (Nasogastric Tube – Jejunum or Duodenum) Feeding and Management Tracheostomy Management Urinary Catheter Management (In-dwelling Urinary Catheter, In-out Catheter, Suprapubic Catheter) Subcutaneous Injections Complex Wound Management Day-to-day management of medications Ventilator management We have certified and experienced medical professionals on board with us who can render community nursing care NDIS programs to all our participants. Once you sign up with us, we allot a medical support staff who will be in touch with you to discuss, monitor, and provide feedback on your medication, treatments, and progress. Besides the NDIS community nursing care services, they will offer advice on how to engage in basic self-care and nursing where in one will become capable of doing simple things like wound care or first aid on their own. 2. Support coordination- to assist participants to achieve their goals. Goals may be NDIS related, for example, to connect with providers, or related to mainstream or informal supports. Coordination of supports can assist participants to access employment or education services or to connect with family, friends and community. Consistent with Prag Consulting’s philosophy and practice, Support Coordinators work with participants and supporters to identify what is most important and the supports required to meet identified goals. Our Support Coordinators & Specialist Support Coordinators can: Help you understand your NDIS plan Identify and negotiate with suitable service providers Assist with the necessary documentation and paperwork Support your choices Assist during crises Work to solve complex systemic barriers to living a good life Our goal is to find you the support you need to live the life you choose so eventually, you won’t need us. 3. Specialist support Coordination- to support participants who are accessing services across multiple areas (for example: NDIS, as well as mental health or justice or child protection). Kindly Care Australia Support Coordinators providing SSC have qualifications, skills and experience in support participants with complex needs. Specialist Support Coordinators work with participants and supporters to identify needs, develop meaningful goals, and ensure supports are in plan to manage challenges and risks to participant safety and well being. Our Specialist Support Coordinators work within an NDIS specialist framework, necessitated by high-level risks present in the participant’s current situation. The support is time-limited and focuses on addressing barriers and reducing complexity in the support environment while assisting the participant to connect with services and build capacity and resilience. It may also involve the development of an intervention plan which will be put in place and supported by Disability or Mental Health support workers. 4. Behaviour Support- We provide specialist behaviour support services to children, young people and adults. Specialised Supported Employment- Supports to assist Participants with employment where it is beyond the requirements of employment services and employers. 6. Assist Access / Maintain Employment - Assistance to Access and maintain employment or higher Education- A special programme for the specially-abled designed to help one find and keep suitable jobs, as well as providing training to help motivate promotions to better roles. Assist Access/Maintain employment services include support associated with career counselling and guidance of which can help them unearth their passions as they discover the career path best suited to them. 7. Assistance With Daily Personal Activities- Daily activities under NDIS helps individuals with disability be as independent as possible in their daily lives. These support services can be given to the participant in his/her own home. It’s easy when you choose us to assist with personal activities of daily living because it’s flexible and can be tailored to meet your own needs, goals, and outcomes. Though these services are usually offered in your own home, they can also be offered in a community environment if the participant wants. At Kindly Care Australia , we offer a personal care assistant who supervises the daily life tasks of our clients to help them learn to live as independently as possible. Assistance In Coordinating or Managing Life Stages, Transitions and Supports- Assistance with life stage transition service includes short and long terms support that aims at strengthening and supporting the participant’s abilities and choices. Transitions such as moving out of one’s home, joining the university, or getting a job are part of everyone’s life. It is never an easy task both emotionally and practically, and it becomes a little more difficult for people with disabilities. Change can be hard and with it comes new environments and settings. Under this NDIS support category, Kindly Care Australia offers a wide range of supports to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting and create a comfortable attitude. However, this doesn’t apply to family members and friends. Innovative Community Participation- to support NDIS participants to build skills to actively participate in their community. Through side-by-side practice, the program involves highly skilled Community Engagement Practitioners who assist participants to expand their opportunities for community participation and employment. Community Engagement Practitioners adopt a capacity-building, community-based approach by working with participants in their local community to increase the likelihood of uptake and successful transition away from the service to informal supports and/or employment. Community Engagement Practitioners maintain a focus on resilience, wellbeing and personal growth to successfully connect to the community, however each service is tailored to best suit the participant, and their goals, to allow them to get the most out of the service. These activities take place in the community and may be: within a group one-to-one with a Kindly Care Australia support worker. Group And Centre Based Activities- We have a range of activities and programs aimed to assist our customers to lead a full and happy life. Social inclusion and interaction programs are specially designed to meet an individual’s need and to assist them in finding their “special place” in life, where they feel valued and comfortable within their surroundings. Kindly Care Australia has Centre-based Activities and Groups to promote socialisation. These Centre-based Activities are facilitated by support workers and have self-directed skill-building activities. We encourage participants to engage in activities independent and/or participate in workshops/group activities.
