Support Coordination Network

Specialist Support Coordination

NDIS Registered

From the provider: The Support Coordination Network was founded in 2012 by a team of people who have accumulated a wealth of knowledge with over a decade of working within the community services sectors and they identified the need to create an innovative, outside of the box and outcomes driven service! We offer support to people living with a disability on a smaller scale basis where an emphasis on providing exceptional person centered care and communication is second to none! We pride ourselves on delivering quality, person centered and goal orientated services that empower you to live your best life. Our Specialist Support Coordinators have a higher level of qualification, skills and experience within a particular area (Housing, Mental Health, Complex Supports, Dual Disability, Child Protection, Justice and Early Childhood Intervention) to provide intensive, usually time limited assistance to help you achieve your goals. An area of expertise is ALL THINGS HOUSING - SDA, MTA, STA, SIL, Respite and innovative living options. We know how complex and difficult it is to navigate finding a new home and transitioning into your new home. We are here to help every step of the way. Our Support Coordinators are friendly and caring professionals who will support you to understand and implement your NDIS plan to achieve your goals. Some tasks we can support you with include find the right services out there in the community who meet your needs, completing referral paperwork, building your capacity (so that perhaps someday you may not need us), money and budgeting, preparing for plan reviews and progress reporting to NDIA. Our Service Locations: We provide services across Australia. We also offer virtual services for participants who are busy or live in remote locations.

TIDE Community Care

Specialist Support Coordination

From the provider: TIDE Community Care is built around Specialist Support Coordination — the centre point where complexity is understood, risks are managed, and systems are brought into order. Our coordination model leads the entire service response, ensuring that NDIS plans translate into real‑world outcomes, providers stay aligned, and participants remain safe, stable, and supported in their homes. From this foundation, TIDE integrates the practical supports that keep people well. Environmental control is our first layer of risk management, delivered through domestic assistance, lawn care, and trauma‑informed or biohazard cleaning when required. These services protect safety, access, and tenancy, forming the groundwork for stability. Personal supports provide daily structure — assistance with activities of daily living, community access, routines, and high‑intensity supports where clinically appropriate. Our clinical services strengthen the system further, offering high‑intensity nursing, assessments, continence management, mental‑health‑informed care, and formal reporting aligned with NDIS and aged care standards. Education underpins everything we do, building capability across clients, families, and support teams so decisions are informed and supports remain sustainable. TIDE is not a list of services — it is a coordinated, clinically informed response that holds under pressure, reduces risk, and restores order for people whose needs exceed standard service models.

Choosing the Right Provider

Finding the right provider is essential for achieving your NDIS goals. Consider these factors:

Experience & Qualifications

Look for providers with relevant experience in your specific support needs and appropriate certifications.

Cultural & Language Fit

South Australia's diverse community means you can find providers who speak your language and understand your cultural background.

Availability & Flexibility

Ensure the provider can accommodate your preferred schedule, whether weekdays, weekends, or evenings.

NDIS Registration Status

NDIS registered providers meet quality and safety standards. Self-managed participants can also choose unregistered providers.

Frequently Asked Questions

How much do Specialist Support Coordination providers cost in South Australia?

Specialist Support Coordination rates in South Australia follow the NDIS Price Guide limits. Costs vary based on the type of support, time of service (standard hours, evenings, weekends), worker qualifications, and travel requirements. Plan-managed and self-managed participants have greater flexibility in choosing providers and negotiating rates.

What's the difference between registered and unregistered NDIS providers?

NDIS registered providers have met quality and safety standards set by the NDIS Quality and Safeguards Commission. Agency-managed participants must use registered providers. Self-managed and plan-managed participants can choose either registered or unregistered providers, giving more flexibility in their choices.

Can I change my Specialist Support Coordination provider if it's not working out?

Yes, you have the right to change providers at any time. Review your service agreement for notice periods and discuss any concerns with your provider first. If you're unhappy with the service, you can end the agreement and find a new provider that better meets your needs.

How do I find Specialist Support Coordination providers who speak my language?

Use the Language filter at the top of this page to find providers offering support in your preferred language. South Australia has providers available in many languages including Mandarin, Cantonese, Arabic, Vietnamese, Greek, Italian, and more. You can also specify language requirements when contacting providers directly.

What qualifications should Specialist Support Coordination providers have?

All NDIS workers must have a valid NDIS Worker Screening Check. Many hold qualifications like Certificate III or IV in Individual Support, Disability, or relevant allied health degrees. Specialist providers may have additional training in specific disability types. Always ask providers about their qualifications and experience.
