A FCA reviews your performance in all areas of daily living around the home, school and community. A FCA report provides detailed information on a person's functional capacity along with the difficulties or limitations they may experience in carrying out every day tasks. A report provides assessment in these areas along with recommendations and strategies to increase your independence. Formal assessments administered by Riverways Of Wellbeing: *Vineland-3 *ADOS-3 *WHODAS 2.0 *Sensory Profile 2 *PEDI-CAT Timeframe: Functional Capacity Assessments can take between 2-3 hours to complete (you or other stakeholders may also be required to complete a Self/Parent/Caregiver/Teacher questionnaire). The report may take between 3-7 hours to complete, depending on the desired outcome required. Riverways Of Wellbeing will then send you and any other requested stakeholders (Local Area Coordinator, Support Coordinator, GP) the report. If you require, our Social Worker can also provide you with ongoing Social Work intervention, therapy, and supports to assist you achieve your goals. Funding/Costs: Riverways Of Wellbeing provide privately funded services and allied heath therapy supports under the National Disability Insurance Scheme (NDIS) for Plan or Self-Managed clients. Social Workers provide Allied Health therapy supports under you Capacity Building Section (Improved Daily Living or Early Childhood Interventions) Direct Therapy/Intervention – $150 per/hr Report Writing – $120 per/hr Telehealth – $130 per/hr Travel – $30 per home visit (if within 30min drive of therapist)
