YP Coordination is an independent NDIS support coordination practice working with plan-managed and self-managed participants on South Australia's Yorke Peninsula. Co-founders Ali and Fallon established it in response to a growing local need, and the practice also runs inclusive community workshops and events open to anyone, with or without disability. Support coordinators help participants understand their plan funding, arrange supports and therapies, organise allied health assessments, and prepare for NDIS reviews. Ali brings more than ten years of experience supporting people with disability, while Fallon has support coordination experience and twelve years in government health.
