Your Link To Assistance is an independent disability and aged care support provider based in Bedfordale, in Perth's south-east, supporting participants across the Perth metropolitan area. The team is small and deliberately personal, built around the belief that care works best when it is warm and familiar. Supports include in-home daily personal activities such as bathing, dressing and meal preparation, supported independent living, in-home and out-of-home respite that gives carers a break, and household and home maintenance tasks including cleaning, gardening, lawn mowing, gutter cleaning and general upkeep.
Capacity building covers development of life skills such as managing finances, cooking and everyday problem solving, assistance with employment or education, and support coordination that keeps services aligned with the participant's plan. Community access is delivered through assistance with travel and transport, community participation, innovative community participation, and group and centre based activities. The team also provides aged care assistance and specialised support for people living with early onset dementia or Alzheimer's, an area where founder Kellie Lean brings lived experience of caring for a family member and particular skill in managing behaviours of concern and maintaining familiar routines.
Referrals are welcome from participants, families, carers and support coordinators through the website's referral form, followed by an initial consultation and a tailored plan.
