A small Tasmanian support coordination practice working with NDIS participants and aged care recipients, along with their families and support networks. The team helps people understand and use their NDIS plan, coordinates a mix of supports, and builds the skills and confidence needed to manage service delivery, maintain relationships and take part in community life. Recovery coaching is also available, connecting participants with health and mental health services outside the NDIS so that supports work together. Staff have backgrounds in the disability and aged care sectors, and the team lives and works locally across Tasmania.
