We collaborate closely with GPs, specialists, allied health teams and support workers to keep your care coordinated and consistent. Navigating the NDIS can be overwhelming — understanding your budget, choosing providers, arranging services and preparing for your plan review all take time and know-how. Our coordinators work alongside you to build your understanding of the scheme, connect you with quality mainstream and community supports, negotiate and set up service agreements, resolve issues as they arise, and strengthen your capacity to manage your own supports over time. Whether you need Support Connection to get started or ongoing Coordination of Supports for a more complex situation, we tailor our involvement to your goals and your funding — and because our team includes qualified allied health professionals, we bring genuine clinical insight to the way your supports fit together.
As your review approaches, we help you gather evidence and articulate your goals so you walk in prepared.
