Southern Rivers Community Services is a not-for-profit organisation, registered as an NDIS provider, that supports people living with disability, people over 65, asylum seekers and culturally and linguistically diverse communities. It works from a head office in the Melbourne CBD and supports participants across Victoria, with self-managed, plan-managed and agency-managed plans all accepted. Supports include core assistance with personal activities, household tasks, gardening, travel and transport in a wheelchair-accessible van, community participation and group activities.
Capacity building covers support coordination at two levels, community nursing by registered and enrolled nurses, assistive technology, supported independent living in SDA properties, and interpreting and translation. Client numbers are kept deliberately small.
