Smart Choice Plan Care provides disability services to NDIS self-managed and plan-managed participants. The services we provide: Support coordination: Developing individual support plans, connecting with service providers, coordinating support services, monitoring service delivery, providing information and education, advocating for participants, building capacity and independence, and reviewing and reporting. Support Worker: 1. Personal Care: Assisting with activities such as bathing, dressing, grooming, and medication management.
2. Household Tasks: Helping with house cleaning, meal preparation, laundry, and other domestic chores. 3. Transportation: Assisting with transportation to medical appointments, community activities, or other destinations. 4. Social Support: Engaging in social activities with individuals, providing companionship, and facilitating community participation. 5. Skill Development: Assisting individuals in acquiring and improving skills such as cooking, budgeting, communication, and problem-solving.
6. Employment Support: Helping individuals with disabilities find and maintain employment, including job searching, resume writing, and workplace accommodations.
