Rise2Shine Care is a disability support provider based in Pakenham, Victoria, working with NDIS participants in Melbourne's south-east and regional communities including Frankston, Shepparton and Traralgon. Support is delivered in the participant's home, in the community and through group programs, with staffing ratios of 1:1, 1:2 or 1:3. The service range covers personal activities and high-intensity personal care, community nursing, household tasks, transport, employment support, community participation, specialist disability accommodation, mental health support and a school holiday program for children and teenagers.
Community nurses coordinate with doctors and allied health professionals, and a separate mental health team works with participants and their support networks on goals, education, training and employment.
