Reignite Support

Accommodation and tenancy assistance covering help to find and secure a shared or private property, moving in, setting up a home, managing tenancy agreements and rental payments, plus supported independent living in the participant's home.

Short term accommodation for up to 28 days a year gives participants and carers a break, with personal care, food, medication assistance and activities. Medium term accommodation covers up to 90 days while other supports are arranged.

One-to-one support with personal care, mobility and transfers, meal preparation, medication and health management, shopping, budgeting, companionship and appointments. Life skills development and life stage transition support are also offered.

Registered nurses provide community nursing in the participant's home, including health care plans, continence and catheter care, wound and stoma care, respiratory support, medication administration and diabetes monitoring.

Support workers provide transport to appointments, events and activities, help with disability-friendly options such as taxis and wheelchair accessible vehicles, travel training on public transport, and travel planning.

A program to build skills and join community events and programs, including help finding suitable employment, support to start a small business, skills workshops, workplace mentoring and self-care.
