Pristine Support Coordination is an NDIS support coordination service for plan-managed and self-managed participants. Support coordinators help participants understand their funding, connect with health, transport and social providers, and keep supports coordinated as needs change. The service also assists with plan reviews and changes, advocacy, and crisis intervention when circumstances shift. Coordination is provided locally and remotely, including by telehealth. The founder brings more than 20 years of healthcare experience, and the practice was set up to offer individualised care to people with disability and older Australians.
