We are a small provider that values a holistic approach to care. We take the time to intentionally match support workers best suited to each client to ensure genuine connection. Our team offers personal care, social support, assistance with daily actives and community access, and offers a broad range of experience working with complex behaviours. We love to collaborate interprofesionally with allied health teams, allowing us to imbed therapy goals into support sessions. Momentum Disability has proudly hosted our local communities first disability specific walking club, in effort to promote social connection & health and wellbeing.
