Link My Plan is an NDIS provider based in Osborne Park, Western Australia, supporting people with a range of disabilities. The organisation matches participants with a peer mentor of similar age and interests, who shares knowledge and experience, encourages problem solving and helps build new skills and connections. Therapy assistants work alongside a participant's allied health team to practise skills and strategies in everyday settings, and support workers help with community activities, household tasks, cooking and other daily activities chosen by the participant.
Recovery coaches spend time with participants and the people important to them, helping them find mental health services, understand the NDIS and act as an advocate. Support coordinators help participants understand their goals, connect with the right providers and build confidence to manage their plan.
