JC Cornerstone Community Care is a Melbourne-based NDIS provider offering support coordination, psychosocial recovery coaching and support work. The organisation was founded by Jacob Cook on lived experience of recovery and works with participants, families, carers and referrers. Support coordination helps participants understand their plans and link with suitable services, while recovery coaching is one-to-one support for people living with psychosocial disability.
Support workers assist with personal care, daily living, community participation and appointments. The team also works with people who have co-occurring alcohol and other drug needs or physical disability, and provides non-clinical NDIS supports only. It operates across Melbourne's inner and south-eastern suburbs and the Mornington Peninsula.
