Interchange Gippsland is a community-based, incorporated disability service organisation working in Gippsland, Victoria, since 1986. It is a registered NDIS provider supporting more than 300 people with disabilities and their families. Services include Level 2 and Level 3 support coordination, one-to-one community access support, adult day programs, social groups, school holiday recreation programs and supported holidays. Support is given at home, in the community and at group venues in Morwell, Bairnsdale and Wonthaggi, for people of all ages with NDIS funding, and programs are built around each person's goals.
Support Coordination
Level 2 support coordination for NDIA-managed, plan-managed and self-managed participants, covering plan implementation, service agreements and connections to other supports.
