Guiding Grace Disability Services offers a number of supports for participants; • Daily living tasks • Community Access • Meal Prep • Personal Care • Transport • Group Activities • Equine Therapy Guiding Grace prides ourselves on being different from other support services, we want to see each precipitant achieve their short and long term goals. We want to be able to work with support coordinators as well as plan manager to make sure we are all on the same page of the participant achieving their goals.
We have a strong focus on the right match with participant and our team. This also carries over into clear and transparent communication with our participant’s in what their needs and wants are. Everything is clearly set out in the participants handbook, as well as the service agreement and budget based on the participants needs for supports as per the service agreement. I have attached a copy of each of these draft documents for your reference.
Currently we are based in Gippsland/Bayside and Mornington Peninsula, We also have a base in Metro Perth and surrounding areas in WA. As we expand we will cover more towns in Victoria and WA. If you have a specific set of requirements and our current team members will not fit those requirements, we are also able to recruit the right skillset for your participant and make sure things run smoothly. We pride ourselves in being different and are more focused on the match between participant and our team.
