Grace Professional Services provide Support Coordination to NDIS participants who have Support Coordination as part of their plan. We are a registered NDIS provider and also provide Specialist Support Coordination. Our team of Support Coordinators are knowledgeable and reliable and will work with you to find the right supports to meet your goals, needs and aspirations. Our job is to help you implement your NDIS plan by coordinating the supports you need and connecting you with disability service providers that will support you.

Based in Frankston, Victoria, Grace Professional Services coordinates supports for NDIS participants throughout Melbourne’s southern suburbs and the Mornington Peninsula.
We will accept clients outside of this region on a case by case basis. Our team has sound knowledge and experience in disability and understands the complexities of the NDIS. Support coordination is all that we do. We can work with you to find the right service providers to help you get the most out of your NDIS plan and make it as stress free as possible. We do not have wait times and can support you quickly. We are a proudly a fully registered provider of the NDIS.
The team is led by CEO and Support Coordinator, Sandi Grace who has many years of experience in disability, home care and Senior Executive roles at a variety of organisations across numerous industries. Sandi also has personal experience of caring for a family member with disability. Sandi has built and trained an incredible team who aim to provide you with the best service and experience possible.
