Community nursing delivered in the home by qualified nurses, covering wound and post-operative care, medication management, continence, catheter and stoma care, diabetes and chronic disease monitoring and seizure management.

Household task support covering cleaning, laundry, dishwashing, deep cleaning and routine home maintenance at a frequency chosen by the participant, plus meal preparation and grocery shopping.

Supported accommodation and tenancy assistance, including group homes, supported living and shared housing, tenancy management, rent in advance, bond, repairs and help with moving.

Short-term accommodation offering a supported place to stay during a crisis or transition, and medium-term accommodation in a group home with staff on site 24 hours a day.

One-to-one support with personal care and self-care, high-intensity care available around the clock including complex wound and bowel care, plus life-stage transition support and daily living skills.

Door-to-door transport to medical appointments, work, education, shopping and community activities, with home pick-ups and drop-offs and wheelchair-accessible vehicles.
