Fraser Coast Support Coordination is an independent support coordination practice based in Torquay, Queensland, working with NDIS participants across Hervey Bay, Maryborough and surrounding areas. The team helps participants understand their NDIS plan, set goals and use their funding, meeting people in their own home or wherever they feel most comfortable. Goal setting, discovery meetings and ongoing individualised sessions form the core of the service, with two support coordinators assigned to each participant. The practice began as a solo venture and has grown into a team whose backgrounds include Centrelink, aged care, financial planning and property.
