Embrace Support provides highly skilled, individualised, and independent Levels 1 and 2 Support Coordination with the purpose of continuing to provide opportunities and independence for people to live their best lives. Our focus is to assist you in coordinating your NDIS funding and support by empowering you with choice, control, dignity, and respect because we know how important your needs and independence are to you. Our Support Coordinators have lived experience with the NDIS and disability and can deliver statewide in Western Australia.
How it works: 1. You are linked to a Support Coordinator based on your needs and goals. They will meet with you in person, online, or over the phone (depending on your location) to discuss and review your NDIS plan and your goals. 2. Your Support Coordinator will work with you to develop an action plan to assist you to achieve your goals and guide you on how to use your plan. 3. Your Support Coordinator will work with you to establish supports that are the best fit for you.
4. Your Support Coordinator will check in with you a minimum of once per month to make sure your NDIS plan and supports are meeting your needs. 5. Your Support Coordinator will assist you to develop the knowledge, skills, and confidence to navigate the NDIS yourself.
