ConneXtions is a charitable not-for-profit disability support organisation formed in 2017 after a two-year co-design process with people with disability in North East Victoria. It delivers plan management, support coordination, personal supports and children's services from offices in Wodonga and Shepparton, with Yarrawonga also served. Children's services cover early childhood intervention and weekly group programs, while personal supports assist with daily tasks at home, school, work and in the community, including overnight support. An employment program serves people over 18 with intellectual disability and autism, and a lawn mowing enterprise covers the Albury-Wodonga region.
Plan Management
ConneXtions pays invoices, claims on the NDIS portal and manages the participant's funding, working with whichever providers the participant chooses. The Careview Advantage app lets participants track their plan.
