Chona Care is an independent NDIS registered provider based in Sydney that specialises in supporting people living with psychosocial disabilities and complex changing medical needs to promote recovery, independence and positive mental health and wellbeing. It offers support in the home and in the community, including help with daily self-care such as showering, dressing and personal care, household tasks like cleaning, meal planning and preparation, and keeping up with appointments and paperwork.
Its team provides transport to medical appointments, work and social outings, and support to take part in community life, including recreational and leisure activities, shopping and bill payments, supported social outings and social groups. Chona Care also works on capacity building with participants, their families and clinical teams, such as building social skills, maintaining a tenancy, connecting with employment agencies and finding and keeping a job.
It provides support in the Eastern Suburbs, the inner city, Sutherland and south-eastern suburbs.
