Choice Community Health supports people with disability through the NDIS. We’re here for people who want more than “support hours”, people who want a life that feels more independent, more connected, and more like their own. We work alongside participants to build everyday skills, confidence, and routines, and to make community access actually possible (not just talked about). That might look like getting out to groups, getting back into hobbies, building friendships, joining local activities, or simply feeling comfortable and capable in day-to-day life.
Our support is practical and person-led. We listen first, then build a plan around what matters to you, your goals, your interests, your culture, your boundaries, and the way you want to live. Some people want help at home; others want help getting out into the community; many want both. Wherever you’re starting from, we aim to support you in a way that builds your capacity over time (not just doing things for you, but doing things with you). Relationships matter to us.
We try to keep support consistent and human, the kind of support where you feel known, safe, and respected. We also believe a good life isn’t built with one “perfect worker”. It’s built with a steady team and a strong sense of belonging, so you’re not left isolated or starting from scratch every time something changes. We support a wide range of disability needs, including people living with complex situations or mental health challenges alongside disability.
Our approach is recovery-oriented and grounded in dignity, choice, and real respect. We take quality and safety seriously, and we want you to feel confident speaking up, asking questions, and giving feedback, because your voice should shape your supports. At the end of the day, our goal is simple: to help you move toward a life that feels fuller, stronger, and more connected, on your terms.
