Community participation programs, outings and skill development workshops that help participants build connections with local organisations and their community.

Household tasks support covering cleaning and organising, laundry, grocery shopping and meal preparation, plus basic home and garden upkeep.

Community nursing delivered in the home and community, including health assessments, medication management, wound care and chronic disease management by a qualified nursing team.

Supported independent living with support plans developed together with the participant, family and support network, covering household tasks, meal preparation and community integration.

Level 2 support coordination that connects participants with providers, makes the most of plan funding, removes barriers and builds capacity to manage an NDIS plan.
