CarePath Support is an NDIS provider delivering disability support from its base in Larapinta, Northern Territory, and in Melbourne. Its services cover hospital discharge care and life skills coaching, community participation, supported independent living in shared homes, support coordination, transport, short term respite, employment support and group programs. Support is shaped around each participant's routines, culture and pace, and the multilingual team brings cultural understanding to its work. The team has worked in the sector for over a decade, with experience in support coordination, and takes referrals from support coordinators, LACs, families and community professionals.

Hospital discharge care combines early planning with short-term in-home personal care, help with daily tasks, transport and home set-up, coordinated with hospital teams for a return home or to transitional accommodation.
