Care Mode Support Services are a registered National Disability Insurance Scheme (NDIS) provider servicing clients in Victoria and New South Wales. We offer a range of support services including: Support Coordination (a team of professional Support Coordinators and Support Workers on hand to assist and coordinate your every need), Accommodation/ Tenancy (STA, MTA, respite care, SDA), Core Supports (Personal care, household tasks, daily tasks assistance, meal prep, community nursing & more), Capacity Building (Increase social & community participation, life skill development, therapeutic supports, sourcing & keeping employment or higher education & more), Allied Health (Occupational Therapy, Speech and Behaviour Support Therapy), Community Nursing (help prevent diseases, maintain health and treat any existing health conditions).
What sets us apart from other providers, to attend to your daily needs and requests, we have a dedicated department of Client Service Officers’ who are the first point of contact should you require assistance with your support and services. Our CSOs work very closely with external stakeholders, liaising and attending meetings with Allied Health and NDIS teams, as well as training staff before commencement of their shifts (if needed). This flexibility means that your supports will suit your specific requirements to ensure you efficiently reach your goals.

DAILY LIVING Through our core home and community care services, Care Mode can assist with the tasks and routine that come about each day.
This allows you to live your best life, on your own terms. Daily living assistance includes: Assistance with daily personal activities to enable to people to live as autonomously as possible - such as personal hygiene, dressing, medication assistance, financial management, communication skills, and lifestyle mentoring. Assistance with daily personal activities (High Intensity) - to enable people with more complex needs and requirements to live autonomously.
Supports are provided by staff with additional experience, skills, and qualifications. Improved Daily Living and Life Skills - development-focused activities and training to enhance the individual's capacity to live as independently in their everyday life. TRANSPORTS Assistance with travel and transport - provision of travel arrangements and support for people to access essential services. COMMUNITY ACCESS Participation in the community – including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills.
This may include: Group and Centre-based activities - such as outings and group and social activities. Innovative Community Participation - includes activities outside the scope of "Participation in the Community" SIL, STA AND RESPITE ACCOMMODATION SERVICES Care Mode provide SIL, STA and respite support that meets the needs of the client. ALLIED HEALTH Our allied health practitioners are trained professionals who can assist you in managing your physical and mental health and wellbeing through services that include prevention, diagnosis, treatment, and rehabilitation. Our services include: Capacity and skill building Mobility and postural support prescription Environmental modifications Home and living supports Functional capacity assessments and therapy recommendations NDIS access assessment and support Behavioural support Speech, communication and language support Mental health supports AND MUCH, MUCH MORE… Our support coordinators will work collaboratively with you to ensure that your lifestyle and needs are never compromised.
Your level of care and inclusions ensure that your support needs are relevant and unique to your requirements. Other types of support include (but are not limited to): Accommodation & Tenancy assistance - the provision of transition supports for people seeking a place to live, which suitably caters to their type and level of needs. Assistance with daily tasks in group or shared living arrangements to help individuals to live autonomously. Assistance with Life Stage Transitions - including mentoring, peer support, individual skill development, and decision making to support people to make transitions on their own terms and in a way that meets their goals and aspirations.
Assistance to access and maintain employment or higher education - including assistance and support with preparation for school leavers to find employment in the supported or open job market, and in their transition through school towards further education.
