AtlaCare Community Supports is a Melbourne-based provider preparing to deliver Level 1 Support Connection and Level 2 Coordination of Supports to plan-managed and self-managed NDIS participants. Our approach is person-centred, practical and guided by each participant’s individual goals, preferences and circumstances. We can help participants understand their NDIS plan, identify suitable services, compare providers, coordinate supports, prepare for plan reassessments and build confidence managing their supports over time.
We can also assist participants to pursue employment, education, community participation, housing and independence goals by connecting them with appropriate NDIS, mainstream and community services. AtlaCare aims to provide clear communication, reliable follow-up and support that respects participant choice and control. We are based in Melbourne and intend to offer both in-person and remote Support Coordination across Victoria. AtlaCare currently has capacity for new plan-managed and self-managed participants across Victoria.
