Community nursing delivered by registered nurses who assess clinical needs with the participant and prepare a care plan. Includes wound care, medication administration, continence and catheter care, stoma and PEG feeding and palliative support.

Assistance to build daily living skills such as using public transport, attending appointments, shopping, cooking, housework, budgeting and paying bills, plus personal care and support through life transitions such as housing and Centrelink.

Shared living support with trained staff for participants seeking to live independently. Individual living arrangements, vacancies and SDA or SIL funding details are not described.

Household cleaning including vacuuming, sweeping, mopping, bathrooms and kitchens, changing bed sheets and laundry, regularly or occasionally. Lawn mowing and pet care can be included, and tradespeople arranged for electrical or carpentry work.

Help preparing healthy meals and assistance with grocery shopping, offered as part of household tasks and daily living support on a regular or occasional basis.

A community participation program built around individual goals, including social groups, library visits, outings, the beach, movies, concerts, sporting clubs, banking and courses. Staff assist with transport to events and with employment skills.
