Advoc8 Health Support Services is a registered NDIS provider delivering in-home disability support, community nursing and support coordination on the Gold Coast and surrounding areas in Queensland. Its support workers provide personal care, daily living support, medication assistance, mobility support and aids, transport, out-of-hours care and community and social participation, working with participants in their own homes and in the community. Registered and enrolled nurses provide community nursing at home, including high intensity supports such as wound care and dressings, medication administration and supervision, continence assessment and catheter care, PEG feeding and enteral nutrition, diabetes and insulin management, stoma care and education, and tracheostomy care.
Support coordination helps participants understand their NDIS plan and budget, connect with and coordinate suitable providers, set up service agreements, prepare for plan reviews and advocate for their needs and goals. The team also offers training and assessment services.
